Faith Formation & Youth Registration Form 2026-2027


We are grateful to God for you and your children! Through the Faith Formation and Youth programs at Our Lady of Grace, we strive to serve you as Jesus would by introducing your children to numerous ways that they may learn about the Catholic faith and build upon their personal relationship with Jesus Christ.

The registration form will soon be added to this site. On this form, you will be able to register your children for classes and youth groups, pay tuition, and volunteer. Our Lady of Grace offers classes and youth activities for children age 4 through high school.

Tuition and Due Dates


Tuition for classes is $75 per child for the year, with a maximum of $225 per family ($10 discount per child if registered by July 1; $5 discount per child if registered by Aug. 31). Scholarships are available. Tuition is WAIVED for children/grandchildren of Catechists and Aides, committing to a particular class/time for the full Faith Formation year. Other consistent volunteer activities may also qualify for waived tuition. Please volunteer in the section near the end of the form.


There is no fee for any student to participate in Middle School Youth Group or Teen Catholic (for high school). There is no fee for Bishop Ryan students to participate in Sacramental Preparation (First Reconciliation and First Eucharist) or Confirmation classes.
